POSITION OVERVIEW:
The Kaplan House (Manhattan) provides therapeutic and support services to adolescents ages 17-21 who are referred to us through ACS and the Juvenile Justice system. Our residents have experienced significant trauma, resulting in behavioral, emotional, and functional/ development issues.  Kaplan House offers an extensive independent living skills program, including vocational preparation, and educational services that assists youth towards reaching independence.

The Case Planner provides support, counseling, case planning and works with the youth towards independent living.
 
KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):
 Client Engagement:
  • Carries a caseload of clients and provides services to those residents within the scope of program regulations and agency mission.
  • Evaluates and assesses independent living skills and supports clients in meeting individual goals.
  • Teach the clients how to advocate for themselves with service providers by modeling advocacy skills.
  • Links clients to community-based treatment and collaborate with other services providers when coordinating services within the Jewish Board as well as with community providers.
  • Practice person-centered and collaborative service planning with clients, also ensuring service plans are completed and signed by client according to regulatory guidelines and standards.
  • Interprets and explains agency policies and procedures to client.
  • Run groups under supervision of the clinical supervisor.
  • Supports residents toward the goal of medication management independence, including appropriate person-centered interventions.
  • Accompanies residents to appointments as needed.
  • Make weekly home visits during business hours as well as after hours as needed.
  • Other tasks as assigned.
 
Team Process:
  • Participates in intake, assessment, discharge, and follow-up conferences with service providers, ACS/ OCFS where needed.
  • Participates in regular staff meetings and training.
  • Travel to offsite meetings and trainings.
 
DOCUMENTATION:
  • Files incident reports as appropriate.
  • Complete paperwork, including case notes, FASPs, PHR’s, transition plans, court reports and all other needed ACS mandated documentation, in a timely manner.
  • Complete monthly home inspection.
  • Participate in audit preparation.
  • Ensures progress notes meet monthly billing documentation requirements, if applicable.

EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):
  • A bachelor’s degree is required, master’s degree in human services or related field preferred
 
ADDITIONAL QUALIFIICATIONS NEEDED:
  • Willingness to work with young adults living with the impact of trauma and experience mental health challenges.
  • Compassion and respect for vulnerable individuals.
  • Patience and understanding for the sometimes-slow process of stabilization and recovery.
 
COMPUTER SKILLS REQUIRED (List the computer skills needed):
  • Familiarity with working in windows, Microsoft word, excel, Teams etc.
  • Ability and willingness to learn new systems and application software.
  • Familiarity with Connections (ACS record), preferred.
  • Familiarity with telehealth.

SALARY: $74,000 -
